Claims can be reported by either of the following methods:
You can report claims to C.L. Frates and Company, the Program Administrator for Chartis Insurance Group, the Élan Insurance Carrier.
Below is a tentative list as to what information and documentation is generally needed to accompany the Proof of Loss (attached below). This general list is as follows:
- A notarized Proof of Loss with supporting information and documentation
- A detailed written narrative giving full particulars as to how the loss was discovered and how the loss occurred (basically, who, what, where, when and why of the claim).
- A copy of the police report (if any) and an update on the status of the investigation/prosecution; if the perpetrator has been sentenced, please provide a copy of that report (if available). Also, please provide the name and address of the prosecutor (if available). The telephone number and name of the detective handling case.
- How you (the Insured) arrived at the claim amount by providing all such financial documentation necessary to support this amount. Where applicable these items should include but not be limited to, copies of checks (both front and back), notes, memos, correspondence, admissions, statements, receipts, ledger or accounting sheets, internal or external auditors report and any addition documentation which the Insured might feel will help support the amount being claimed on the Proof of Loss. Also, please indicate how the Carrier should interpret all documents submitted in support of claimed loss.
- Please provide copies of the signed merchant fraud letter noting the date and amount of the transactions for total loss amounts for each card holder.
- Please provide documentation showing that the transactions were posted to the customer accounts.
- Please provide information regarding your internal investigation into the fraud allegations.
- Please provide documentation showing that the customers were credited back the loss amounts.
- Please provide documentation the bank exercised any applicable charge back rights.
- Please provide documentation for the number of cards reissued (if any), the cost associated with the reissue and documentation showing payment was made for the reissue of the cards
- Whether you have recovered any portion of the loss, and information as to the likelihood of recovery.
- Provide us with the changes that have been made internally and externally to prevent a recurrence of this type of claim.
